Welcome to Year 1 (7th grade)

Below you’ll find the documents and files needed to complete year two of Holy Spirit’s Confirmation Program (which takes place outside of classes), along with their due dates.

DECEMBER 3:

• Confirmation information form due 

• Baptismal certificate due

JANUARY 23:

• Sponsor eligibility form (Sealed & Signed by their Pastor) due 

• Sponsor interview due 

• Family Prayer form due 

MARCH 4:    

• Service project forms due

• Saint report due

• Theology of the Body Parent Letter  

All paper forms can be handed to your child's School of Religion (SOR) teacher or Mr. Leikam. If any new forms or links to online forms are needed, please contact [email protected]

You can turn in your assignments in one of three ways.

• Hand them to your SOR teacher.

• Hand them to Angelina Creedon in the SOR office

• Email all of your assignments to [email protected]
